About the Role

We are seeking a dedicated and compassionate Reablement Worker to join our team at Wiltshire Council. As a Reablement Worker, you will play a vital role in supporting individuals in their homes post-hospital discharge, helping them regain their independence and achieve their goals.

Key Responsibilities

  • Provide personal care and support to customers, facilitating daily living activities and promoting independence.
  • Work closely with occupational therapists to develop personalized reablement plans, ensuring customers receive tailored support.
  • Contribute to daily communications, accurate record keeping, and incident reporting, maintaining customer confidentiality and security of information.
  • Participate in regular team meetings, customer review meetings, and staff development programs, assuming responsibility for own professional development.
  • Assist customers with mobility aids, assistive equipment, and moving and positioning, adhering to health and safety requirements and organizational policies.

Requirements

  • Care Certificate or equivalent skills and experience, with a willingness to undertake required training.
  • General level of education to GCSE A*-C or equivalent skills and experience.
  • Empathy with the elderly and/or people with a disability, and an understanding of their needs.
  • Ability to work alone and as part of a team, prioritizing and organizing own workload.
  • Pleasant and helpful manner, effective communication both written and oral.

What We Offer

As a Reablement Worker at Wiltshire Council, you will receive a competitive salary, a £650 welcome payment upon starting, and a further £650 upon successful completion of your probation (subject to tax and NI).
