Children's Community Nurse Specialist

We are seeking a skilled and compassionate Children's Rapid Response Nurse to join our team at Birmingham Community Healthcare NHS Foundation Trust. As a key member of our Children's Rapid Response Nursing Teams, you will provide high-quality, specialist nursing care to children and families in their own homes, preventing unnecessary hospital admissions and promoting greater choice and control over their care.
Key Responsibilities:
- Work autonomously and independently within our policies, procedures, and guidelines to provide evidence-based therapeutic interventions.
- Collaborate with multiple professionals across organizations and sectors to ensure comprehensive assessment, development, implementation, and evaluation of care.
- Provide expert nursing care to children with complex healthcare needs, life-limiting/threatening illnesses, and end-of-life care.
- Work closely with families and carers to develop and implement personalized care plans that meet their unique needs and preferences.
