Job Title: Children's Nurse - Community Children's Nursing

An exciting opportunity has arisen to join our established team of Community Children's Nurses in Birmingham on a permanent basis. This is a city-wide service that offers specialist nursing support to children and families with a variety of complex physical health needs.

Main Duties:
  1. Work in conjunction with the Children's Rapid Response Service to prevent hospital admissions and support children and families with life-limiting conditions.
  2. Provide comprehensive, evidence-based nursing care to children within their own homes and other community settings.
  3. Assess, plan, and deliver holistic care to children and families, utilizing an agreed nursing model.
  4. Work in partnership with children, families, and carers to provide optimum individual outcomes, acknowledging their personal preferences and lifestyles.
  5. Liaise and collaborate with acute services, primary healthcare teams, and other agencies to plan and implement care packages.
  6. Provide clinical leadership in care packages funded by the Community Complex Care team.
  7. Provide health education information to children, families, and carers to promote healthy lifestyles through health promotion activities.
  8. Plan and participate in teaching packages to families and carers to ensure evidence-based care is delivered.
  9. Advise the Team Leader, Clinical Lead, or Service Manager of any concerns related to patient care or concerns raised by primary healthcare teams or partner agencies.
  10. Undertake home visits to children registered with GPs within Birmingham as required.
  11. Undertake home visit risk assessments to ensure safety during visits, especially during unsociable hours.
  12. Obtain informed consent from children, parents, and carers prior to undertaking care.
  13. Maintain clear, concise, and contemporaneous records, adhering to BCHC record-keeping policies.
  14. Be professionally and legally accountable for nursing practice and all activities.
  15. Provide a flexible service to accommodate the working environment.
Person Specification:
  • Essential:
    • RSCN/RN Child Branch
    • ENB 998 (Teaching & Assessing in Clinical Practice) or equivalent
    • Community Children's Nursing qualification or equivalent
  • Desirable:
    • Children's Palliative Care experience
    • Independent supplementary prescriber or equivalent
Experience:
  • Essential:
    • Significant experience of working as a Children's Nurse either RSCN/RN Child
  • Desirable:
    • Experience of working with children with life-limiting/threatening illnesses and end of life
Skills/Knowledge:
  • Essential:
    • Ability to manage a team on a day-to-day basis in the absence of the Team Leader
    • Demonstrate knowledge relating to child protection issues and procedures
    • Evidence of assessment skills and use of nursing process and nurse models
    • Demonstrate sound use and application of NMC Code of Conduct and scope of professional care
    • Demonstrate knowledge of current legislative changes affecting NHS and Social Services, particularly those affecting community care
    • Demonstrate knowledge and application of current clinical trends and practices, inclusive of evidence-based practice in relation to Children's Community Nursing
    • Demonstrate ability to teach relatives, patients, clients, and staff across all areas of self-care and appropriate nursing practice
    • Demonstrate knowledge of standard setting and audit processes
    • Demonstrate individual responsibility in respect to health and safety
    • Ability to demonstrate written skills that are concise, accurate, relevant, and legible
    • Awareness of diversity of the needs of families in relation to culture and religion
    • Provide evidence of time and resource management
    • Excellent communication skills
