About the Role

We are seeking an experienced mental health professional to join our Crisis Resolution and Home Treatment Team (CRHTT) as a Community Mental Health Practitioner. The successful candidate will work with individuals experiencing acute mental health concerns, providing urgent assessments and intensive community-based treatment services as an alternative to hospital admission.

Key Responsibilities
  • Undertake urgent assessments of individuals with mental health problems in acute crisis and provide intensive community-based treatment services as needed.
  • Work collaboratively with the multi-disciplinary team to provide safe and effective treatment in the community, facilitating early discharges from acute inpatient units.
  • Develop and implement care plans, working closely with service users, their carers, and colleagues to manage risk and promote recovery.
  • Contribute to the development and implementation of team systems and processes, including outcome measures and audit.
Requirements
  • Post-registration experience with at least some experience at band 6.
  • Experience in acute and community areas, with a focus on mental health nursing or social work.
  • Evidence of continuing professional development, including training in care planning, change management, and Microsoft Office.
  • Knowledge of physical health contributions to mental health issues, difficulties, and assessment.
