Job Summary:

As a key member of our team, you will be responsible for providing high-quality assessment and intervention services to older adults with mental health problems or dementia.

Key Responsibilities:

  • Conduct comprehensive assessments of older adults with mental health problems or dementia, identifying their needs and developing personalized care plans.
  • Deliver evidence-based interventions, including occupational therapy sessions, to promote independence, well-being, and quality of life.
  • Collaborate with multidisciplinary teams, including healthcare professionals, social workers, and family members, to ensure seamless care coordination.
  • Develop and maintain strong relationships with community partners, including local authorities, voluntary organizations, and other healthcare providers.
  • Participate in quality improvement initiatives, contributing to the development of best practices and policies that enhance patient care and outcomes.

Requirements:

  • Registration with the Health and Care Professions Council (HCPC) as an Occupational Therapist.
  • Postgraduate qualification in Occupational Therapy or a related field.
  • Experience working with older adults with mental health problems or dementia, preferably in a community setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Strong problem-solving and analytical skills, with the ability to prioritize tasks and manage multiple responsibilities.
