Job Summary

We are seeking a skilled and dedicated Community Health Nurse to join our team at NHS Scotland. As a key member of our Community Nursing workforce, you will be responsible for delivering high-quality, person-centered care to housebound individuals within the community.

Key Responsibilities
  • Deliver effective and efficient care to patients in their homes, working in partnership with service users, other core members of the Home Team, and aligned services.
  • Manage a caseload population derived from all GP practices within the Stewartry, providing comprehensive care and support to individuals with complex needs.
  • Travel independently to remote areas of the region, utilizing a driving license to access patients in rural and hard-to-reach areas.
Requirements
  • A current UK NMC registration and ability to travel independently, outwith public transport routes/times, is essential.
  • Strong communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare professionals.
  • Ability to work autonomously and as part of a team, prioritizing tasks and managing time effectively to meet deadlines.
