Job Title: Community Staff Nurse

We are seeking a skilled and dedicated Community Staff Nurse to join our team in the Westside area. As a Community Staff Nurse, you will be responsible for providing high-quality nursing care to patients in their own homes.

About the Role:
  • Provide nursing care to patients in their own homes, working closely with other healthcare professionals to ensure comprehensive care.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Conduct regular assessments and evaluations to monitor patient progress and adjust care plans as needed.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
  • Participate in ongoing education and training to stay up-to-date on best practices and new developments in community nursing.
Requirements:
  • Registered Nurse with current registration and a valid UK driving license.
  • Experience working in a community nursing setting, with a strong understanding of community health needs and priorities.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong commitment to delivering high-quality patient care.
  • Flexibility to work a variety of shifts, including evenings and weekends, as required.
