Community Health Nurse

This is an exciting opportunity to join the established Community Nursing workforce and be part of the Stewartry Home Team. As a Community Health Nurse, you will deliver high-quality, effective, person-centered care to housebound individuals within the community, working in partnership with service users, other core members of the Home Team, and aligned services including care providers.
Key Responsibilities- Deliver high-quality, effective, person-centered care to housebound individuals within the community.
- Work in partnership with service users, other core members of the Home Team, and aligned services including care providers.
- Develop and maintain effective relationships with service users, their families, and carers.
- Contribute to the development and implementation of care plans that meet the individual needs of service users.
- Participate in multidisciplinary team meetings to discuss service user care and share best practice.
- A current UK NMC registration and ability to travel independently, outwith public transport routes/times, is essential.
- A current driving license is essential, but consideration will be given if alternative transport can be arranged.
- Ability to work in a fast-paced environment and prioritize tasks effectively.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
