About the Role

We are seeking a highly skilled and compassionate Social Work Assistant to join our Family Support Team at The Hospice of St. Francis.

As a key member of our team, you will provide psychosocial support to individuals and families affected by life-limiting illnesses, advocating for their emotional and practical needs.

Your main duties will include completing assessments of patients' and families' needs, collaborating with our multi-professional team, and referring to external agencies and providers when necessary.

You will also have the opportunity to develop your skills in group facilitation, training, and service audit and evaluation.

About You

  • Minimum of three years' experience in Social Care
  • Social Care qualification or similar
  • Experience of working in a multi-disciplinary team
  • Ability to work collaboratively with our partners in the NHS, Social Care, and Charitable sector
