Job Description:

We are seeking a highly skilled Care and Support Specialist to join our team in North Ayrshire. This is an exciting opportunity to make a positive impact on the lives of carers and their families.

About the Role:

The successful candidate will be responsible for establishing links within community health settings across North Ayrshire, identifying carers and their families who need information, advice, and support. They will work closely with hospital staff, social work, and other teams to ensure that unpaid carers are informed, involved, and supported throughout discharge planning processes for loved ones.

The role involves providing a direct service to individual carers, informing them of their rights under the Carers Act (Scotland) 2016, and facilitating carer involvement in discharge planning of patients. The successful candidate will also be responsible for regular check-ins with carers throughout the hospital stay and following the point of discharge.
