Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a Staff Nurse, you will be responsible for providing high-quality patient care to individuals with complex needs, including palliative care. You will work collaboratively with a multidisciplinary team to deliver patient-centred care, promoting independence and dignity.

Main Duties
  • Provide high-quality patient care to individuals with complex needs, including palliative care.
  • Work collaboratively with a multidisciplinary team to deliver patient-centred care, promoting independence and dignity.
  • Develop and implement individualized care plans, taking into account the patient's physical, emotional, and social needs.
  • Conduct regular assessments and reviews to ensure the patient's needs are met and their care plan is up-to-date.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Person Specification
  • Current NMC registration, level 1.
  • Experience of working within a multidisciplinary team.
  • Experience of working with patients with multiple pathologies and developing and delivering care plans designed to meet their needs.
  • Excellent verbal, written, and interpersonal communication skills.
  • Basic computer skills.
  • Ability to maintain and improve standards of care.
  • Experience of audit.
  • Up-to-date knowledge of current clinical/professional issues.
  • Understanding of resource management.
  • Knowledge and understanding of learner needs.
