We are seeking an experienced Support Assistant Practitioner to join our team in Norwich, providing vital support to individuals and families in their health and wellbeing journey.
About the Role
This hybrid role combines elements of both Support Assistant Practitioner and Assistant Practitioner positions, offering the successful candidate a unique opportunity to develop their skills and experience in a dynamic and fast-paced environment.
Your Key Responsibilities
- Provide high-quality support to patients and families, ensuring a smooth and safe discharge process.
- Collaborate with healthcare professionals, social care teams, and community resources to deliver effective support and services.
- Develop and maintain strong relationships with stakeholders, promoting open communication and trust.
Requirements and Qualifications
- A minimum of 1 year's experience in Health or Social Care, with a good standard of education and excellent IT skills.
- Competent at working at pace in responsive, fast-paced environments, with a positive can-do attitude.
- Ability to work independently or as part of a team, with good listening and analytical skills.
