Job Overview

In this role as a Clinical Pharmacist, you will be an integral part of the General Practice team in Norfolk Primary Care Ltd, working closely with pharmacists and clinicians to improve patient health outcomes and efficiency. The post holder will provide direct, accessible, and timely medicines expertise, including face-to-face contact with patients.

Key Responsibilities
  1. Provide pharmaceutical consultations to patients with long-term conditions as part of the multidisciplinary team.
  2. Work in partnership with pharmacists and clinicians in Acute General Hospitals to improve prescribing safety and quality after discharge from hospital admissions.
  3. Improve prescribing quality through clinical audit and education, aligning performance with NICE standards and clinical guidance.
Requirements
  • Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
  • Experience with common acute and long-term conditions seen in general practice.
  • Ability to communicate complex information effectively and follow legal, ethical, and professional policies.
Benefits
  • $49,500 salary range (dependent on qualifications and experience).
