Job Title: Paediatric Nurse Case Load Lead

We are seeking a skilled and experienced Paediatric Nurse to lead our case load team in the North East London / Essex area. As a Paediatric Nurse Case Load Lead, you will be responsible for providing face-to-face support to children and their families, while creating personalised care plans for each child.

The ideal candidate will have a minimum of 6 months' experience in the UK, an active NMC PIN, and up-to-date experience of tracheostomy care and ventilators within the last 2 years. They will also possess strong communication skills, both verbally and written, to liaise effectively with patients, families, and the multi-disciplinary team.

Key Responsibilities:

  • Design and deliver tailored care plans specific to individual client needs
  • Effectively manage the MDT through strong communication and teamwork
  • Provide ongoing monitoring, support, and assessment of clients' physical and physiological needs within your caseload
  • Deliver nursing interventions, including the management of medicine delivery
  • Build and maintain sophisticated relationships with clients' families
  • Work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care
