Job Description

We're looking for a Clinical Case Manager with experience in Inpatient Bedside Nursing for both Adults and Paediatrics to join our Case Management Team at Hca Cancer Network.

Responsibilities:
  1. Work closely with various healthcare professionals to assess, plan, implement, and evaluate care options and services that meet patients' health needs.
  2. Collaborate with healthcare professionals to coordinate proactive comprehensive discharge planning activities.
  3. Act as a resource/liaison between patient, hospital, physician, and third-party payers to optimise patients' community treatment/services.

The successful candidate will have a strong background in nursing, with experience in utilisation review and discharge planning. Excellent communication, organisation, and computer skills are also essential.
