Job Description:

We are recruiting for a Therapeutic Support Professional to work with our Adult Social Care Team in Surrey. As a key member of our team, you will be responsible for assessing, planning, and implementing occupational therapy interventions for adult patients.

Main Responsibilities:
  1. Conducting Assessments: Evaluate patients' functional abilities, occupational needs, and environmental factors to inform treatment plans.
  2. Developing Treatment Plans: Create tailored plans based on assessment findings and patient-centred goals.
  3. Providing Therapeutic Interventions: Deliver hands-on support to patients, their families, and carers to promote adaptive skills and daily living activities.
  4. Collaborating with Healthcare Professionals: Work closely with doctors, nurses, physiotherapists, social workers, and carers to ensure coordinated care.
  5. Maintaining Patient Records: Accurately document patient progress and adjust treatment plans as needed.
Requirements:
  • HCPC registration
  • 2 years of experience in Social Services
  • Valid driver's license
Salary and Benefits:
  • Estimated salary: £40.00 per hour
  • 36 hours worked per week, with a mix of home working and office/home visits
