Job Overview

We are seeking a skilled and compassionate Care Coordinator to join our Psychiatric Intensive Care Team at Pennine Care NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to our service users.

The successful candidate will have a strong understanding of mental health difficulties and a passion for delivering meaningful activities that promote recovery and well-being. You will work collaboratively with our team to design and deliver ward-based activity programs that cater to the diverse needs of our service users.

Main Duties
  • Plan, organize, and deliver ward-based activity programs on an individual and group basis.
  • Provide specialist support to patients and work collaboratively with the multidisciplinary team.
  • Adopt a non-judgmental approach with patients, based on respect and dignity.
  • Undertake ward-based activities that promote recovery, are valued, and increase independence.
  • Contribute to the development of an overall ward-based activity program that promotes involvement and choice.

Our team provides short-term intensive assessment, treatment, and support to service users who may be experiencing a mental health crisis. We work in a trauma-informed way, utilizing the RAID model of care, to maximize service user recovery and promote a least restrictive approach.

Person Specification
  • NVQ Level 3 Health and Social Care or equivalent short courses and/or experience.
  • PMVA Qualification or commitment to complete (Trust-funded).
