Care Coordinator

We are seeking a skilled and compassionate Care Coordinator to join our Psychiatric Intensive Care Team at Pennine Care NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care and support to individuals experiencing mental health crises.
Our team provides short-term intensive assessment, treatment, and support to service users who may be experiencing complex mental health needs. We work in a trauma-informed way, utilizing the RAID model of care, and strive to provide care that is person-centered and recovery-focused.
Main Duties- Plan, organize, and deliver ward-based activity programs on an individual and group basis.
- Provide specialist support to patients and work collaboratively with the multidisciplinary team.
- Adopt a non-judgmental approach with patients, based on respect and dignity.
- Undertake ward-based activities that promote recovery, are valued, and aim to increase independence.
- Contribute to the development of an overall ward-based activity program that promotes involvement and choice.

Person Specification- NVQ Level 3 Health and Social Care or equivalent qualifications and experience.
- PMVA Qualification or commitment to complete.
- Evidence of further education or qualifications in areas such as art, music, or reminiscence.
- Recent experience of working in healthcare or a related area.
- Experience of coordinating and running group activities.
- Experience of working with a team in a healthcare or social care setting.
- Understanding of dementia and other mental health difficulties.
- Knowledge of the importance of meaningful activity to promote well-being.
- Ability to design suitable activities to meet patient needs.
- Able to work independently and responsibly using own initiative.
- Able to provide clear verbal instructions and present written information clearly.
- Able to conduct risk assessments for groups and activities.
- Able to complete tasks and manage time effectively.
- Flexible to suit the needs of the service.
