Job Overview

We are seeking a skilled and compassionate Care Coordinator to join our Psychiatric Intensive Care Team at Pennine Care NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to our service users.

The successful candidate will have recent experience of working in healthcare or a related area, with a strong understanding of dementia and other mental health difficulties. You will be able to design and deliver engaging activities that promote well-being and independence, and work collaboratively with our multidisciplinary team to achieve the best possible outcomes for our service users.

Main Duties and Responsibilities
  • Plan, organize, and deliver ward-based activity programs on an individual and group basis.
  • Provide specialist support to patients and work collaboratively with the multidisciplinary team.
  • Adopt a non-judgmental approach with patients, based on respect and dignity.
  • Undertake ward-based activities that promote recovery, are valued, and increase independence.
  • Contribute to the development of an overall ward-based activity program that promotes involvement and choice.
  • Design, manage, and promote health improvement programs for patients.
  • Support medical and nursing staff in the completion of physical health monitoring.

Our team provides short-term intensive assessment, treatment, and support to service users who may be experiencing a mental health crisis. We work in a trauma-informed way, utilizing the RAID model of care, and aim to work in the least restrictive way possible while maintaining patient safety.

Person Specification
  • NVQ Level 3 Health and Social Care or equivalent short courses and/or experience.
  • PMVA Qualification or commitment to complete (Trust-funded).
Desirable Criteria
  • Evidence of further education or qualifications, e.g., art, music, reminiscence.
Experience
  • Recent experience of working in healthcare or a related area.
  • Experience of coordinating and running group activities.
  • Experience of working with a team within a healthcare, social care, or related setting.
Knowledge
  • Understanding of dementia and other mental health difficulties.
  • Knowledge of, and ability to design suitable activities to meet patient needs.
Skills and Abilities
  • Able to work independently and responsibly using own initiative.
  • Able to provide clear verbal instructions and present written information clearly.
  • Able to conduct risk assessments for groups and activities.
  • Able to complete a task and manage time.
  • Flexible to suit the needs of the service.
  • Confidence to facilitate group activities independently.
  • Ability to motivate older people with mental health problems.
  • Effective and flexible communication skills.
  • A willingness to learn and develop self and practice.
