Job Description:

We are seeking a skilled Health Visitor to join our team at Hunter AHP. As a Health Visitor, you will play a vital role in promoting health and well-being within the community. Your primary responsibility will be to manage caseloads and deliver the Healthy Child programme, ensuring exceptional care for families and individuals.

Key Responsibilities:

  • Conduct maternal mood assessments and new birth visits to identify potential health risks and provide support to new mothers.
  • Champion child safeguarding and promote overall well-being by working closely with families, community groups, and other healthcare professionals.
  • Collaborate effectively within multi-agency settings to ensure seamless care and support for families and individuals.

Requirements:

  • Postgraduate Diploma or Certificate in SCPHN Health Visitor or equivalent qualification.
  • Current registration with the Nursing and Midwifery Council (NMC).
  • Proficient in computerized recordkeeping and Excel, with experience with Rio system an advantage.
  • Strong communication and interpersonal skills, with the ability to work effectively in diverse teams.
  • Commitment to continuous professional development and staying up-to-date with best practices in health visiting.
