Role Overview
NHS CityCare Partnership is seeking an experienced Nurse Practitioner to join their Community Care team. As a key member of the team, you will work closely with patients and their families to provide high-quality personalized assessment and care in their own homes.
The successful candidate will have a strong understanding of complex physical and mental health needs, with the ability to develop and implement goal-focused rehabilitation programs for adults. You will also be responsible for coordinating safe and timely discharges from community settings, including community beds.

Key Responsibilities
- To provide high-quality personalized assessment and care to patients in their own homes
- To develop and implement goal-focused rehabilitation programs for adults with complex physical and mental health needs

- To risk assess, advise, and provide equipment as necessary
- To participate actively in training requirements and continuing professional development
Salary: £37,338-£44,962 per year pro rata for part-time work
