Community Care Professional

Livewell Southwest is seeking a Community Care Professional to join our team in Plymouth. This is a fantastic opportunity to make a meaningful impact in the lives of individuals with severe and enduring mental health problems. As a key member of our community care services, you will work closely with our multi-disciplinary team to provide holistic, evidence-based interventions that promote recovery and resilience.
About the Role:
This role requires a highly skilled and compassionate professional who can work effectively with service users, their families, and other stakeholders to develop personalized care plans. You will be responsible for maintaining accurate clinical records, participating in mandatory training, and engaging in reflective practice to continually improve your skills and knowledge. Your ability to work collaboratively with other statutory and VCSE organizations will be essential in building strong relationships and driving positive outcomes for our service users.
Key Responsibilities:
You will be responsible for working with service users and their families to develop care plans that address their unique needs and goals. You will also contribute to the co-production of care plans in line with the Personalised Care Framework, ensuring that our services are tailored to meet the evolving needs of our clients. Additionally, you will be expected to manage your caseload independently, maintaining accurate electronic records and scheduling appointments as needed. Your ability to work autonomously while receiving guidance from registered professionals will be essential in delivering high-quality support to our service users.
Requirements:
To be successful in this role, you will need relevant experience of working with people with severe and enduring mental health problems and their carers. Your understanding of mental health and social care legislation and your ability to work within these frameworks will also be crucial. A minimum of 5 GCSEs grades A-C, including maths and English, is required. Additionally, your ability to follow instruction from qualified practitioners, keyboard skills, and safeguarding knowledge will be essential in this role.
Salary and Benefits:
The salary for this role is £38,000 - £45,000 per annum, depending on experience.
