Job Description

We're seeking a talented Community Support Professional to join our team In this role, you'll play a vital part in delivering high-quality care services to our clients, helping them to maintain their independence and quality of life. You'll work closely with our multidisciplinary team to assess client needs, create tailored support plans, and deliver care services that meet or exceed their expectations.

As a Community Support Professional, you'll have the opportunity to work with a diverse range of clients, each with unique needs and requirements. You'll be responsible for conducting home visits, providing emotional support, and helping clients with daily tasks and activities. You'll also work collaboratively with our team to ensure seamless delivery of care services and continuous improvement of our support programs.

Key Responsibilities:
  • Conduct home visits to assess client needs and provide emotional support
  • Create tailored support plans to meet client needs and preferences
  • Deliver high-quality care services that meet or exceed client expectations
  • Work collaboratively with the multidisciplinary team to ensure seamless delivery of care services
  • Monitor and evaluate client progress, making adjustments to support plans as needed
Requirements:

To succeed in this role, you'll need to possess excellent communication and interpersonal skills, with the ability to build strong relationships with clients, colleagues, and other stakeholders. You should also have experience working in healthcare or social care, preferably in a similar capacity. A valid first aid certificate and a DBS check are also essential prerequisites for this position.
