About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at CJP Outreach Services. As a Positive Behaviour Support Worker, you will play a vital role in providing person-centred support to adults with complex behavioural needs and communication difficulties.

Key Responsibilities
  • Provide individualised support to enable individuals to develop essential life skills and independence.
  • Collaborate with individuals to create and implement tailored support plans, incorporating Positive Behaviour Support strategies and interventions.
  • Assist individuals with daily living tasks, such as cooking, personal hygiene, and budgeting.
  • Support individuals with medication management, medical appointments, and maintaining good health and well-being.
  • Encourage participation in community activities, social events, and access to local services.
  • Maintain accurate and comprehensive care records, documenting progress, incidents, and outcomes.
  • Work collaboratively with colleagues, families, and external professionals to ensure holistic care.
Working Arrangements

We provide 24/7 support with staffing ratios of 1:1 and 2:1. You will work on a rotational basis, including every other weekend, with sleep-in duties available.
