About the Role

At PeoplePlus, we are seeking a skilled and compassionate Social Care Advisor to join our team. As an Independent Living Advisor, you will provide expert guidance and support to individuals with disabilities, enabling them to live independently in their local communities.

Your Key Responsibilities

  • Work closely with customers, health professionals, and social care teams to understand their needs and develop personalized support plans.
  • Assess customers' abilities and create effective strategies to promote independence and confidence.
  • Collaborate with internal teams to ensure seamless delivery of services and excellent customer outcomes.

Requirements

  • Relevant experience in social care, preferably with a background in working with vulnerable adults or children.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with customers and stakeholders.
  • Strong organizational and time management skills, with the ability to prioritize tasks and meet deadlines.
  • A driving license and access to a vehicle.
