About the Role

We are seeking a highly skilled and experienced Children's Community Nurse to join our team at Dorset County Hospital NHS Foundation Trust. As a key member of our Children's Community Nursing Team, you will play a vital role in providing high-quality nursing care to children and young people with complex health needs.

As a Children's Community Nurse, you will work closely with children, young people, and their families to assess, plan, implement, and evaluate individualized care plans. You will also provide training and support to families and other healthcare professionals to empower them to provide high-quality care.

The ideal candidate will have significant experience in children's nursing, with a strong background in assessing and managing complex health needs. You will be able to work autonomously and as part of a team, with excellent communication and interpersonal skills. A teaching qualification and evidence of continuous professional development are also essential.

Key Responsibilities
  • Assess and manage complex health needs in children and young people
  • Develop and implement individualized care plans in collaboration with children, young people, and their families
  • Provide training and support to families and other healthcare professionals
  • Work closely with other healthcare professionals to ensure seamless care transitions
  • Participate in on-call rota for Children's Community Nursing Team
Person Specification
  • Registered Children's Nurse (RSCN/RCN)
  • Teaching Qualification or equivalent
  • Evidence of continuous professional development
  • Significant experience in children's nursing
  • Ability to work in a flexible manner and prioritize according to the demands of the situation
