About the Role

We are seeking a highly skilled and experienced Children's Community Nurse to join our team at Dorset County Hospital NHS Foundation Trust. As a key member of our Children's Community Nursing Team, you will play a vital role in providing high-quality nursing care to children and young people with complex health needs.

As a Children's Community Nurse, you will work closely with children, young people, and their families to assess, plan, implement, and evaluate individualized care plans. You will also work collaboratively with other healthcare professionals to ensure seamless care transitions and optimal health outcomes.

Key Responsibilities:

  • Conduct comprehensive assessments of children's health needs and develop individualized care plans
  • Provide high-quality nursing care to children and young people with complex health needs
  • Work collaboratively with other healthcare professionals to ensure seamless care transitions and optimal health outcomes
  • Support and supervise junior staff to ensure safe and effective care delivery
  • Participate in on-call rota for Children's Community Nursing Team

Requirements:

  • Registered Children's Nurse (RSCN/RCN) qualification
  • Teaching qualification or equivalent
  • Evidence of continuous professional development
  • Willingness to undertake further education in relation to Children's Community Nursing
  • Holds a degree in a relevant health-related subject
