About the Role

We are seeking a skilled and compassionate Community Care Support Worker to join our team at NHS Professionals Limited. As a Community Care Support Worker, you will play a vital role in providing high-quality care and support to patients in their own homes and communities.

Key Responsibilities
  • Monitor patients' conditions and provide regular updates to healthcare professionals.
  • Assist patients with daily living activities, such as bathing, dressing, and medication management.
  • Provide emotional support and companionship to patients and their families.
  • Work collaboratively with healthcare teams to develop and implement personalized care plans.
  • Maintain accurate and detailed records of patient interactions and care provided.
Requirements
  • Minimum of 6 months paid experience in a community environment within the last 2 years.
  • Comply with NMC Professional Registration and keep up to date with current nursing practices.
  • Valid UK driving license and access to a car.
