Job Title: Community Diabetes Specialist Dietitian

We are seeking a highly skilled and experienced Dietitian to join our team at County Durham and Darlington NHS Foundation Trust. As a Community Diabetes Specialist Dietitian, you will play a crucial role in providing expert dietetic care to patients with diabetes in our community settings.

About the Role

This is an exciting opportunity for a motivated and enthusiastic Dietitian to join our team. You will work closely with our multidisciplinary team to provide comprehensive care to patients with diabetes, including those with complex needs. Your expertise will be essential in developing and implementing individualized care plans, providing education and support to patients and their families, and collaborating with other healthcare professionals to ensure seamless care.

Key Responsibilities
  • Provide expert dietetic care to patients with diabetes in community settings
  • Develop and implement individualized care plans in collaboration with patients and their families
  • Provide education and support to patients and their families on diabetes management and self-care
  • Collaborate with other healthcare professionals to ensure seamless care and coordination
  • Contribute to the development and implementation of local and national strategies and policies related to diabetes care
Requirements
  • Registered with the Health Care Professions Council
  • Member of the British Dietetic Association
  • Experience of working in a variety of clinical settings, including community and acute care
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
