Community Health Practitioner

We are seeking a skilled and compassionate Community Nurse to join our team at Oxford Health NHS Foundation Trust. As a Community Nurse, you will have the opportunity to deliver high-quality, patient-centered care in the comfort of patients' own homes.
Main DutiesAs a Community Nurse, you will be responsible for providing holistic care to patients with complex health needs. This will involve working closely with patients, their families, and other healthcare professionals to develop and implement personalized care plans.
You will have the opportunity to work in a variety of settings, including patients' homes, clinics, and community centers. You will be supported to develop your skills and expertise through our comprehensive induction program and ongoing training and development opportunities.
Person SpecificationWe are looking for a qualified and experienced Community Nurse who is passionate about delivering high-quality care. The ideal candidate will have:
- Registered General Nurse (adult) qualification
- Education to degree level or equivalent
- Knowledge of long-term conditions and their management
- Ability to manage competing priorities and work under pressure
