Job Summary

The Single Bed Hub is responsible for receiving and processing referrals for patients requiring a discharge to assess (D2A) bed-based services. This involves liaising with staff, patients, and families to ensure a safe and timely bed transfer, ensuring that all IT systems are up-to-date.

Main Responsibilities
  • To support the tracking of patients' journeys from referral to discharge in a timely manner using in-house CDT IT systems and Excel, ensuring that all time frames are met.
  • Support the Allocation Team to manage bed capacity across all areas to ensure that all capacity is reported and utilized.
  • Working with the team to ensure that bed allocation is appropriate and in line with the service criteria.
  • Support the team to manage the daily unmet demand - including attending daily meetings to discuss LOS and flow across all acutes with service leads.
  • Develop relationships with all partners and colleagues to support timely updates and flow through discharge to assess beds.

Person Specification

This role involves close working with health and social care staff and an understanding of community Home First services alongside knowledge of the Care Act 2014 and the principles of Discharge to Assess (D2A) would be desirable. The role will involve close working with Track and Triage teams across Staffordshire and involves sharing written and verbal information promptly and politely. This is a seven-day-a-week service as such some weekend working and Bank Holiday cover will be involved.

Qualifications

  • oEducated to NVQ Level 2 standard or equivalent level of knowledge and experience.
  • oGCSE English grade C or above or equivalent experience.
  • oICT Qualification to the competency level of ECDL/CLAIT etc.

Experience

  • oExperience of working within a hospital or community setting and have an understanding of Patient Flow.
  • oExperience of using computer-based information systems in an office environment.
  • oAbility and experience of using IT systems including Microsoft Office and have accurate and competent keyboard skills.
  • oExperience in work planning, and prioritizing.

Skills

  • oStandard IT literate with evidenced experience of relevant IT systems/databases.
  • oHave an understanding of Patient Flow and how this affects the Local Health Economy, both within a Hospital and Home setting.
