About the Role

We are seeking a highly experienced and dedicated professional to fill the position of Reablement Service Manager. As a key member of our team, you will play a vital role in guiding individuals with complex needs through their Reablement journey.

Job Requirements
  • Salary: £40,000 - £50,000 per annum, depending on experience.
  • Location: London (with flexibility to work across a 7-day schedule).
  • Job Type: Full-time.
Key Responsibilities
  • Care Coordination & Caseload Management: Manage a diverse caseload, coordinating Reablement care plans for service users with complex needs.
  • Assessments & Compliance: Conduct social work-specific assessments in line with Safeguarding Adults procedures and the Mental Capacity Act.
  • Multi-agency Collaboration: Work in a multi-agency context, sharing information, and participating in joint assessments and care planning.
  • Risk Management: Collaborate with service users, carers, and professionals to minimize and manage risks.
  • Safeguarding & Advocacy: Promote and safeguard the welfare of vulnerable adults.
  • Record Keeping & Reporting: Maintain comprehensive case records and reports.
  • Participation & Development: Attend and contribute to multidisciplinary meetings, supervision, and training.
  • Health & Safety Compliance: Adhere to the Council's Health and Safety Policy and relevant legislation.
