About the Role

We are seeking a highly skilled and experienced Senior Discharge Care Coordinator to join our team at Hertfordshire Partnership University NHS Foundation Trust. As a key member of our Enhanced Discharge Team, you will play a vital role in ensuring seamless transitions for patients from acute inpatient services to community care.

Key Responsibilities
  • Develop and maintain effective relationships with multidisciplinary teams, including clinicians, social workers, and community services.
  • Conduct thorough assessments to identify patient needs and develop personalized care plans.
  • Collaborate with patients, families, and caregivers to ensure informed decision-making and empowerment.
  • Facilitate communication between healthcare providers, community services, and patients to ensure timely and safe discharges.
  • Monitor and evaluate patient progress, making adjustments to care plans as needed.
Requirements
  • Registered mental health nurse or registered social worker with relevant experience in mental health services.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Experience with electronic recording systems and ability to produce accurate, timely reports.
