Position Overview

Are you a qualified mental health nurse, occupational therapist, or social worker? We present an exceptional opportunity for a Lead Care Transition Specialist.

Are you keen on collaborating with a distinguished NHS Trust to provide community-based mental health treatment?

The Lead Care Transition Specialist serves as a crucial link between acute inpatient care and community support services within Adult Acute and Urgent Care Services.

This role is part of the supportive Enhanced Discharge Team / Integrated Patient Flow Team, supervised by a bed manager and clinician, ensuring daily operational support.

The Lead Care Transition Specialist can utilize various 'hot desk' facilities, promoting flexible working arrangements.

Through established connections with the Integrated Patient Flow Team, the Specialist will coordinate with Discharge Coordinators and Housing Workers to facilitate actions for service users transitioning from community teams in Adult Acute and Urgent Care Services.

Key Responsibilities

The successful candidate will be expected to:

  • Hold a valid registration as a mental health nurse with NMC or as a social worker with Social Work England.
  • Possess relevant experience in mental health services.
  • Participate in inpatient bed flow meetings and collaborate closely with Discharge Coordinators to identify discharge barriers.
  • Assess potential delays in discharge processes.
  • Communicate anticipated outcomes from admissions with Care Coordinators and duty community teams.
  • Oversee daily reviews of discharge readiness for service users.
