Job Summary

We are seeking a highly skilled and experienced Senior Discharge Care Coordinator to join our team at Hertfordshire Partnership University NHS Foundation Trust. As a key member of our Enhanced Discharge Team, you will play a vital role in ensuring seamless transitions for patients from acute inpatient services to community care.

About the Role

The successful candidate will be responsible for providing expert care coordination, facilitating communication between healthcare professionals, and ensuring that patients receive the necessary support to achieve optimal outcomes. You will work closely with our Integrated Patient Flow Team, Discharge Coordinators, and Housing Workers to progress actions for service users and promote efficient care transitions.

Key Responsibilities
  • Provide expert care coordination and facilitate communication between healthcare professionals.
  • Ensure seamless transitions for patients from acute inpatient services to community care.
  • Work closely with the Integrated Patient Flow Team, Discharge Coordinators, and Housing Workers to progress actions for service users.
  • Develop and implement effective care plans to promote optimal patient outcomes.
  • Collaborate with multidisciplinary teams to ensure comprehensive care and support for patients.
Requirements
  • Registered mental health nurse with NMC or registered social worker with Social Work England.
  • Relevant experience in mental health services, with a strong understanding of care coordination and patient flow.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to analyze complex information and make informed decisions.
  • Strong problem-solving skills and ability to work under pressure.
