Job Summary

We are seeking a highly skilled and experienced Senior Discharge Care Coordinator to join our team at Hertfordshire Partnership University NHS Foundation Trust. As a key member of our Enhanced Discharge Team, you will play a vital role in ensuring seamless transitions for patients from acute inpatient services to community care.

About the Role

The successful candidate will be responsible for providing a high level of support to patients, families, and carers, ensuring that their needs are met and that they receive the best possible care. You will work closely with multidisciplinary teams, including clinicians, social workers, and other healthcare professionals, to coordinate discharge planning and ensure that patients receive the necessary support and resources to maintain their independence.

Key Responsibilities
  • Coordinate discharge planning for patients, including identifying barriers to discharge and developing strategies to overcome them.
  • Work closely with multidisciplinary teams to ensure that patients receive the necessary support and resources to maintain their independence.
  • Provide high-level support to patients, families, and carers, ensuring that their needs are met and that they receive the best possible care.
  • Develop and maintain effective relationships with community services, including social services, housing, and voluntary sector organizations.
  • Contribute to the development and implementation of policies and procedures related to discharge planning and care coordination.
Requirements

To be successful in this role, you will need to have:

  • A relevant professional qualification, such as a nursing or social work degree.
  • Extensive experience in healthcare, preferably in a discharge planning or care coordination role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, carers, and healthcare professionals.
  • Strong analytical and problem-solving skills, with the ability to identify and overcome barriers to discharge.
  • Knowledge of relevant legislation, policies, and procedures related to discharge planning and care coordination.
