About the Role

We are seeking a highly skilled and experienced Senior Discharge Care Coordinator to join our team at Hertfordshire Partnership University NHS Foundation Trust. As a key member of our Enhanced Discharge Team, you will play a vital role in ensuring seamless transitions for patients from acute inpatient services to community care.

Key Responsibilities
  • Provide expert coordination and liaison between inpatient services and community teams to facilitate timely discharges.
  • Work closely with multidisciplinary teams to identify and address barriers to discharge, ensuring patients receive optimal care and support.
  • Develop and maintain effective relationships with service users, carers, and healthcare professionals to promote person-centered care and improve outcomes.
  • Contribute to the development and implementation of discharge plans, ensuring they are tailored to individual needs and preferences.
  • Collaborate with colleagues to identify opportunities for service improvement and contribute to the development of innovative solutions.
Requirements
  • Registered mental health nurse or registered social worker with relevant experience in mental health services.
  • Proven track record of working in a discharge coordination role, with a strong understanding of the complexities involved.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with diverse stakeholders.
  • Ability to work autonomously and as part of a team, with a flexible approach to meet the needs of the service.
  • Knowledge of relevant legislation, policies, and guidelines, with a commitment to upholding the highest standards of care and practice.
