Job Overview: We are looking for a skilled Mental Health Care Coordinator to join our team at Look Ahead Care Support and Housing in Slough. The successful candidate will have experience in providing support services to individuals with mental health needs, enabling them to live independently and achieve their goals.

Key Responsibilities:
• Undertake key-working responsibilities for a caseload assigned by the Team Leader/ Manager
• Conduct initial and continuous assessments of needs and potential risks, agreeing levels of support and actions
• Create support/action plans for providing appropriate services based on assessment and reflecting available resources
• Support customers to undertake domestic tasks wherever possible, including practical assistance where necessary
• Develop and maintain links with all key agencies and service providers in the local community
• Empower customers to ensure they receive the services and benefits they are entitled to
• Carry out H&S and Fire Safety checks in individual rooms and communal areas
• Identify and report Safeguarding concerns promptly
• Maintain accurate and comprehensive tenant/customer records to professional standards
• Engage in learning and development activity to increase knowledge and skills
