About the Role

We are seeking a highly motivated and dynamic staff nurse to join our North Tees & Hartlepool Community Nursing Service. This service provides care to patients in a range of settings, including their own homes and residential care homes.

Main Responsibilities

The successful candidate will have a level of knowledge in the field of community care and a specific interest in areas such as palliative care, long-term conditions, and wound care. They will be required to work autonomously to provide a whole person approach to the care and management of patients within this setting.

  • Palliative and End of Life Care
  • Wound care
  • Diabetes management
  • Continence management
Requirements

The ideal candidate will have:

  • RGN qualification
  • Current community nursing experience
  • Evidence of working as part of a multidisciplinary team
Working Arrangements

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

The salary for this post is £29,970 to £36,483 per annum, pro rata.
