Job Title: Apprentice District Nurse

We are seeking a highly motivated and dynamic staff nurse to join our team at North Tees & Hartlepool Community Nursing Service. This role will provide care to patients within a range of settings, including patients' own homes and residential care homes.

Main Responsibilities:
  • Support patients in their own homes, including residential care homes, to deliver high-quality care.
  • Work autonomously to provide a whole person approach to the care and management of patients within this setting.
  • Link closely with the local authority and other specialist nursing teams to provide individualized and holistic care.
Requirements:
  • RGN qualification.
  • Excellent communication skills.
  • Experience of working as part of a multidisciplinary team.
Desirable:
  • Teaching or mentorship qualification.
  • Current community nursing experience.
  • Use of SystmOne.
