Role Description:
As a Care Coordinator - Mental Health, you will be responsible for coordinating care services for individuals living with mental ill health, intellectual disability, autism, and addictions. You will work closely with service users, families, and healthcare professionals to ensure seamless delivery of support.

**Estimated Salary:** £28,000 - £32,000 per annum (dependent on location and experience)

Key Responsibilities:
* Develop and implement individualized care plans in collaboration with multidisciplinary teams
* Coordinate referrals to specialist services and advocate for service user needs
* Foster strong relationships with service users, families, and external partners to ensure effective support

Requirements:
* 3+ years of experience in care coordination or a related field
* Excellent communication and organizational skills
* Ability to work effectively in a fast-paced environment
