About The Role

The Inclusion Service at Educators Wales works with children with additional needs and disabilities, their families, and settings/schools to ensure that children access their entitlement with smooth transitions into a range of settings.

We are seeking a highly organised, motivated, and passionate individual to join our dynamic team as an ALN Transition Worker (Placements). The successful candidate will focus on working with parents/carers to ensure the transition into specialist provision is as smooth as possible for children with additional needs and disabilities.

Key Responsibilities

  • Work with parents/carers to ensure a smooth transition into specialist provision for children with additional needs and disabilities.
  • Monitor and support schools/settings to successfully include all children with additional needs and disabilities to access their entitlement.
  • Participate in partnership working to improve outcomes for children.

Requirements

  • Highly organised, motivated, and passionate individual.
  • Ability to work with parents/carers and schools/settings to achieve successful outcomes for children.
  • Safeguarding and Child Protection knowledge and skills.

Working Arrangements

This is a full-time position, 37 hours per week across 52 weeks (non-term time). The post is temporary until 31st August 2025.
