About the Role

We are seeking a highly motivated and compassionate Community Outreach Support Worker to join our team at Longhurst Group. As a Community Outreach Support Worker, you will play a vital role in providing support and guidance to individuals with enduring mental health needs, enabling them to live independently and achieve their goals.

Key Responsibilities
  • Maximising Independence: Work with individuals to identify and achieve their goals, promoting independence and autonomy in all aspects of life.
  • Housing Related Support: Provide support with accessing benefits, paying rent and household bills, securing accommodation, and accessing employment, volunteering, and training opportunities.
  • Personal Care and Social Inclusion: Assist with daily living tasks, such as shopping, medication management, and domestic chores, while promoting social inclusion and community engagement.
  • Record Keeping and Risk Assessment: Maintain accurate records of support provided, including risk assessments and support plans.
  • Collaboration with Partner Agencies: Work closely with partner agencies, such as local authorities, NHS services, and other community organizations, to ensure comprehensive support for individuals.
Requirements
  • Experience in Mental Health Support: Ideally, you will have experience working in a similar role, supporting individuals with enduring mental health needs.
  • Knowledge of Mental Health: A good understanding of mental health issues and the challenges associated with them is essential.
  • Communication and Interpersonal Skills: Excellent communication and interpersonal skills are required to build strong relationships with individuals, families, and partner agencies.
  • Driver's Licence: A full driver's licence is essential, as you will be required to use a fleet of pool cars to carry out your role.
