Overview

We are seeking a compassionate and skilled Street Healthcare Outreach Nurse to join our team at Change Grow Live. As a key member of our outreach services, you will provide high-quality clinical care and support to individuals experiencing homelessness.

Key Responsibilities

  • Deliver healthcare interventions to individuals in need, working closely with our outreach teams to identify and address health needs.
  • Develop and implement innovative clinical interventions to improve health outcomes for our service users.
  • Collaborate with our multidisciplinary team to provide comprehensive support and care to individuals experiencing homelessness.

Requirements

  • Registered Nurse with a passion for working with vulnerable populations.
  • Experience in healthcare outreach or a related field.
  • Strong communication and interpersonal skills.
