Key Responsibilities- Assess, diagnose, and manage patients with undifferentiated diagnoses, providing high-quality care and treatment.
- Develop and implement treatment plans, taking into account patient needs and preferences.
- Prescribe and review medication, ensuring effective and safe treatment.
- Work collaboratively with other healthcare professionals to provide comprehensive care.
- Participate in quality improvement initiatives, contributing to the development of policies and procedures.
- Provide education and support to patients, promoting healthy lifestyles and self-care.
- Manage and lead on the delivery of specific services or projects, ensuring effective outcomes.
- Registered Nurse with a postgraduate diploma or degree in Advanced/Autonomous Practice.
- Qualified Nurse Prescriber with experience in prescribing.
- Experience working in primary or urgent care environments, with a focus on chronic disease management.
- Proficiency in EMIS Web and other relevant clinical systems.
