About the Role
We are seeking an experienced and skilled Social Prescribing Link Worker to join our team at Kettering & South West Rural Primary Care Network. As a Social Prescribing Link Worker, you will play a vital role in supporting patients to access community resources and services that can help them manage their health and wellbeing.
Key Responsibilities
- Work with patients to identify their needs and develop personalized care plans
- Coordinate and liaise with multiple stakeholders to meet specified outcomes
- Provide advice and signposting to patients
- Undertake quality improvement activity
- Manage own workload and prioritize tasks effectively
Requirements
- Professional clinical qualifications or competencies commensurate to a Band 7 AfC appointment
- GCSE or equivalent grade C level qualification in Maths and English
- Minimum of one year's experience of working with healthcare professionals and/or previous experience in the NHS or social care or relevant field
