Job Description:

We are seeking an experienced and skilled Community Nurse to join our team at Cambridgeshire & Peterborough NHS Foundation Trust. The successful candidate will have a passion for delivering high-quality patient care and be able to work autonomously as part of a multidisciplinary team.

The primary role of this position is to provide nursing intervention within patients' own homes, with access to specialist and advanced practitioners when required. This includes working at part of the Neighbourhood team which includes District/Community Nurses, Physiotherapists, Occupational Therapists, Mental Health Nurses, and Team coordinators.

Key Responsibilities:

  • Assess and formulate care plans for patients
  • Provide a high standard of nursing intervention within patients' own homes
  • Work collaboratively with other healthcare professionals to ensure comprehensive care
  • Support and educate peers, new staff, non-registered staff, and students

Salary: £30,000 - £40,000 per annum (dependent on experience)
