Job DescriptionMedication Management:As a Clinical Pharmacist, you will play a critical role in optimizing therapeutic outcomes through effective medication management. This will involve conducting regular medication reviews to ensure safe and effective use of medications, managing drug interactions, and participating in multidisciplinary teams to optimize patient care.
You will be responsible for ensuring that prescription requests are processed accurately and efficiently, monitoring for potential drug interactions and contraindications, and resolving medication issues and requests from hospital correspondence.
Your expertise will be invaluable in completing pharmacy tasks on a daily basis, including dispensing medications, managing stock levels, and maintaining accurate records.
Patient Care and Counselling:As a key member of our healthcare team, you will provide comprehensive counselling to patients on medication use, side effects, interactions, and storage. You will also assist healthcare providers in making informed clinical decisions regarding pharmacotherapy, utilizing up-to-date clinical data and evidence-based guidelines.
You will participate in the management of chronic diseases by providing expertise in pharmacotherapy, contributing to patient care plans, and monitoring patient progress. Your ability to communicate complex information clearly and succinctly will be essential in engaging with patients and healthcare professionals to ensure optimal care.
Collaboration and Communication:You will work closely with a multidisciplinary team, including doctors, nurses, and other healthcare professionals, to ensure a coordinated approach to patient care. Your strong communication skills will enable you to effectively discuss treatment plans and medication information with patients and healthcare professionals.
Quality Improvement and Compliance:You will participate in quality assurance programs to maintain high standards in pharmacy services and patient care. You will also ensure that all pharmacy activities and procedures comply with relevant laws, regulations, and standards, including GDPR, clinical governance, and professional pharmacy practice.
