**Salary**: £29,485 - £31,088 per annum, inclusive of benefits
We are seeking an experienced Adult Care Coordinator to join our team at Great Ormond Street Hospital for Children NHS Foundation Trust.
In this role, you will be responsible for coordinating the care of adult patients who require ongoing support and services.
You will work closely with our healthcare professionals to ensure that patients receive comprehensive and compassionate care, tailored to their specific needs.
The successful candidate will possess excellent organizational skills, be able to communicate effectively with patients, families, and colleagues, and demonstrate a caring and compassionate approach to care delivery.
