PCN Advanced Nurse Practitioner

Job Summary

Principal Medical Limited is seeking a highly skilled and experienced Advanced Nurse Practitioner to join our team at the PML/Banbury Cross Health Centres (BCHC) in Oxfordshire. The successful candidate will be responsible for delivering high-quality, patient-centred care to our patients, working independently and as part of a multidisciplinary team.

Key Responsibilities

  • Provide comprehensive assessments and diagnoses to patients in all settings, including the home environment.
  • Develop and implement individual care management plans, prioritising patient needs and working collaboratively with the healthcare team.
  • Prescribe and review medication as necessary, adhering to PGD and within the scope of practice.
  • Liaise with external services and agencies to ensure patients receive appropriate support.
  • Supervise and mentor junior staff, promoting professional development and knowledge sharing.
